Actress Demi Moore has checked out of an exclusive rehabilitation center in Utah and is now on vacation at an undisclosed location, sources told E! Online.
Moore checked into the Cirque Lodge facility in Sundance, Utah, in early February after she was rushed to a Los Angeles hospital semi-conscious and convulsing on Jan. 23, having allegedly smoked an illegal substance.
The rehabilitation facility is "considered one of the nation's finest drug treatment centers providing services for alcohol and substance abuse," according to its website.
It is believed 49-year-old Moore was smoking illegal K2 spice, a synthetic cannabis, prior to being hospitalized.
Sources said Moore was continuing to take time out of the spotlight following completion of treatment -- reportedly for addiction issues and an eating disorder -- at the rehab facility.
"She's in no rush to get back to L.A.," the source told E! Online. "She's on total lockdown and only talking to a small group of people."
Moore announced in November that she was ending her marriage to Ashton Kutcher after six years, amid reports that her 33-year-old husband was unfaithful.
Moore and Bruce Willis -- who is the father of Moore's three daughters, Rumer, 23, Scout, 20, and Tallulah, 18 -- were married from 1987 to 2000.
According to the entertainment website, Tallulah has been staying with Willis while Moore seeks help.
The "Ghost" star previously was married to rock musician Freddy Moore from 1980-85.
